2024 BFA Summer Camp
Volunteer Application
Thank you for applying as Camp Staff for this summer's BFA Junior Balloonist camp. Camp Staff have significant contact with campers and their personal information therefore they are required to be over 18 and complete a BFA Background check, any additional background checks as required per state and host location(s) and BFA Youth Protection training. Please complete the application below. If selected as Camp Staff you will be contacted by the Camp Director to complete this process. Please read and understand the BFA Youth Program Protection Policy on the website before completing this application. Background Check Consent - Disclosure of Information
I hereby give consent for an investigative consumer report to be done on me for employment or tenant purposes. I hereby authorize, without reservation, any state motor vehicle department, law enforcement agency, state agency, state repository, credit agency, city, state, federal court contacted by the BFA or designated company to furnish any and all information. I do understand the investigation will include information from law enforcement agencies, state agencies and public records information, such as credit, social security, criminal, motor vehicle and workers' compensation in accordance with the American with Disabilities Act. This report will include information as to my character work habits, performance and experience, along with the reasons for termination of past employment from previous employers. This releases the aforesaid parties from any liability and responsibility for collecting the above information at any time.
According to the Fair Credit Reporting Act (Law 91-508) SS 606: A person may not procure or cause to be prepared an investigative consumer report on any consumer unless it is clearly and accurately disclosed to the consumers that an investigative consumer report including information as to his character, general reputation, personal characteristics and mode of living and employment history, whichever are applicable, may be made.
I also understand that if I am denied employment because of the consumer investigation, it is my right to have the name of the agency or agencies disclosed to me within the time allowed. This authorization, in original or copy form, shall be valid for this and any further reports or updates that may be requested.

BFA Youth Program Volunteer Participation Release and Waiver
I hereby, waive, give up and release the Balloon Federation of America (BFA) and the BFA Junior Balloonist Program from any and all claims arising from any injuries, accidents, illness or other mishaps occurring to me during my volunteer activities as a volunteer during the camp. My intention is to release and hold harmless all the parties mentioned above and any parties involved with the camp from any claim which I may have in the future regardless of it's cause arising from activities which I decide to undertake as a BFA Youth Program camp volunteer.
Photo Release
I hereby grant the Balloon Federation of America and BFA Junior Balloonist Program (Listed Entities) the right to take photographs, videos and/or audio recording of the volunteer and further grant the Listed Entities and their assignees and transferees to copyright, use and publish the same in print and/or electronically without any compensation in connection with the above-identified event, including for example such purposes as publicity, illustration, advertising, social media, associated websites and web content.
